If you would like to know if we are a provider for your insurance, or if we accept your insurance, please call the number on the back of your insurance card to verify.
We do not accept Medicare and Medical Assistance. We gladly accept: Cash, Checks, Mastercard, Visa, American Express and Discover.
We provide a 5% discount to patients who do not have dental insurance but pay their amount due the day of their appointment if paid by cash or check.
For your convenience we now offer online credit card payments. Register on www.patientpaycenter.com then go to "make a payment". After you register, confirm your registration from the email you will receive; then come back to the site to start the payment process.
Go to "Make a Payment" and in the box enter the from below for the provider statement code. Confirm the payment is for Dahl and Mack Dental. Add the required information for payment and confirm. This site is secure and will not store your credit card information. You will receive an email receipt.

Dahl & Mack Dental will be changing the way we mail billing statements to our patients.
Billing will be done based on the following schedule:
The first Wednesday of each month – last names beginning with “A” through “G” will be billed
The second Wednesday of each month – last names beginning with “H” through “M” will be billed
The third Wednesday of each month – last names beginning with “N” through “S” will be billed
The fourth Wednesday of each month – last names beginning with “T” through “Z” will be billed
The statements will now have a “Due Date” printed on them in the top right corner. Your statements will show charges that have been completed and if the charges were sent to your dental insurance. If charges are pending with your dental insurance you will not receive a statement until the insurance claim has been processed, indicating what insurance did or did not cover. Any charges remaining after insurance would be the patient’s responsibility – indicating that payment would be due by the due date printed on the statement. If payment is not received by the due date, a late payment charge of $10.00 will be added to your statement each month.
